    Alexander von Humboldts Beschreibungen von Vulkanen in seinen Reiseberichten (Reise auf dem Río Magdalena, durch die Anden und Mexico) zeigen sowohl das Zutrauen zu als auch die Ungeduld mit literarischen Konventionen und Reiseerzählungen. Indem Goethes „Italienische Reise“ und Bürgers „Münchhausen“ als Vergleichsstoffe für literarische Darstellungen der Vulkanbesteigung herangezogen werden, untersucht der Aufsatz Humboldts Schreibprozeß, um moderne Formen von Abbildung und Metapher zu finden. ; Alexander von Humboldt’s descriptions of volcanic mountains in his travel journals (Reise auf dem Río Magdalena, durch die Anden und Mexico) show both his reliance on and impatience with literary conventions and travel narratives. Using Goethe’s Italienische Reise and Bürger’s Münchhausen as points of comparison for literary treatments of the volcano ascent, Humboldt’s process of writing is examined. Humboldt shows the failure of the existing discourse and begins to experiment with narratives which fragment and recombine personal and historical modes of writing with, in this case, images from new technical inventions which visualize landscape according to fundamental scientific principles. While the inclusion of scientific prose is relevant, Humboldt’s link to modernity is based on experimental narrative techniques which draw upon changing sets of discourse practices to describe complex realities. ; Las descripciones de Alexander von Humboldt de las cadenas montañosas de formación volcánica en su relación de viaje evidencian tanto confianza como impaciencia con respecto a los relaciones de viaje de otros cronistas tomados como modelos literarios. En la comparación con “El viaje italiano“, de Goethe, y con “Münchhausen“, de Bürger, como ejemplos literarios de ascensiones a volcanes, se analiza el proceso de escritura de Humboldt para buscar formas modernas de ilustración y metáfora.

    OBJECTIVES: The purpose of this work was to establish the normal range of maximal renal hyperemic response in humans and to identify the ideal renal vasodilatory stimuli. BACKGROUND: Stenotic renovascular atherosclerosis is increasingly treated by percutaneous transluminal renal intervention but with an unpredictable outcome. This may be due to hemodynamically non-significant stenosis or the presence of irreversible damage to the glomerular circulation. We propose that the renovascular hyperemic response may help identify appropriate patients. METHODS: In 28 normotensive patients, quantitative angiographic measurements of the renal artery were obtained, and renal artery pressure and flow velocity were continuously recorded after various hyperemic agents. RESULTS: In a first group of 11 patients, a significant increase in renal artery average peak velocity (APV) was observed after intrarenal (IR) bolus injection of 600 μg isosorbide dinitrate (41 ± 19%), 30 mg papaverine (50 ± 34%), 50 μg dopamine (94 ± 54%), 0.8 μg·kg-1 fenoldopam (80 ± 25%), and during IR infusion of 1 μg·kg-1·min-1 fenoldopam (86 ± 28%). A second group of 17 patients received intravenous infusion of dopamine (3, 5, 10, 20, 30, and 40 μg·kg -1·min-1). The 3 and 5 μg·kg -1·min-1 of dopamine modestly reduced renal resistance index (RI) (-13 ± 15% and -25 ± 20%, respectively). At higher dosages, no further decline in RI was observed. No significant change in vessel diameter was observed before and after the administration of the pharmacological stimuli suggesting that changes in APV corresponded with changes in absolute renal blood flow. CONCLUSIONS: The normal renal flow reserve averages approximately 2 in humans with normal renal function. An IR bolus injection of 50 μg·kg-1 of dopamine is the most convenient means to elicit maximal renal hyperemia. © 2006 by the American College of Cardiology Foundation.

    International audience ; EPIC-Heart is the cardiovascular component of the European Prospective Investigation into Cancer and Nutrition (EPIC), a multi-centre prospective cohort study investigating the relationship between nutrition and major chronic disease outcomes. Its objective is to advance understanding about the separate and combined influences of lifestyle (especially dietary), environmental, metabolic and genetic factors in the development of cardiovascular diseases by making best possible use of the unusually informative database and biological samples in EPIC. Between 1992 and 2000, 519,978 participants (366,521 women and 153,457 men, mostly aged 35-70 years) in 23 centres in 10 European countries commenced follow-up for cause- specific mortality, cancer incidence and major cardiovascular morbidity. Dietary information was collected with quantitative questionnaires or semi-quantitative food frequency questionnaires, including a 24-h dietary recall sub-study to help calibrate the dietary measurements. Information was collected on physical activity, tobacco smoking, alcohol consumption, occupational history, socio-economic status, and history of previous illnesses. Anthropometric measurements and blood pressure recordings were made in the majority of participants. Blood samples were taken from 385,747 individuals, from which plasma, serum, red cells, and buffy coat fractions were separated and aliquoted for long-term storage. By 2004, an estimated 10,000 incident fatal and non-fatal coronary and stroke events had been recorded. The first cycle of EPIC-Heart analyses will assess associations of coronary mortality with several prominent dietary hypotheses and with established cardiovascular risk factors. Subsequent analyses will extend this approach to non-fatal cardiovascular outcomes and to further dietary, biochemical and genetic factors.

    International audience ; BACKGROUND: Carotid stenting is less invasive than endarterectomy, but it is unclear whether it is as safe in patients with symptomatic carotid-artery stenosis. METHODS: We conducted a multicenter, randomized, noninferiority trial to compare stenting with endarterectomy in patients with a symptomatic carotid stenosis of at least 60%. The primary end point was the incidence of any stroke or death within 30 days after treatment. RESULTS: The trial was stopped prematurely after the inclusion of 527 patients for reasons of both safety and futility. The 30-day incidence of any stroke or death was 3.9% after endarterectomy (95% confidence interval [CI], 2.0 to 7.2) and 9.6% after stenting (95% CI, 6.4 to 14.0); the relative risk of any stroke or death after stenting as compared with endarterectomy was 2.5 (95% CI, 1.2 to 5.1). The 30-day incidence of disabling stroke or death was 1.5% after endarterectomy (95% CI, 0.5 to 4.2) and 3.4% after stenting (95% CI, 1.7 to 6.7); the relative risk was 2.2 (95% CI, 0.7 to 7.2). At 6 months, the incidence of any stroke or death was 6.1% after endarterectomy and 11.7% after stenting (P=0.02). There were more major local complications after stenting and more systemic complications (mainly pulmonary) after endarterectomy, but the differences were not significant. Cranial-nerve injury was more common after endarterectomy than after stenting. CONCLUSIONS: In this study of patients with symptomatic carotid stenosis of 60% or more, the rates of death and stroke at 1 and 6 months were lower with endarterectomy than with stenting.     A text parsing component designed to be part of a system that assists students in academic reading an writing is presented. The parser can automatically add a relational discourse structure annotation to a scientific article that a user wants to explore. The discourse structure employed is defined in an XML format and is based the Rhetorical Structure Theory. The architecture of the parser comprises pre-processing components which provide an input text with XML annotations on different linguistic and structural layers. In the first version these are syntactic tagging, lexical discourse marker tagging, logical document structure, and segmentation into elementary discourse segments. The algorithm is based on the shift-reduce parser by Marcu (2000) and is controlled by reduce operations that are constrained by linguistic conditions derived from an XML-encoded discourse marker lexicon. The constraints are formulated over multiple annotation layers of the same text.

    Discourse segmentation is the division of a text into minimal discourse segments, which form the leaves in the trees that are used to represent discourse structures. A definition of elementary discourse segments in German is provided by adapting widely used segmentation principles for English minimal units, while considering punctuation, morphology, sytax, and aspects of the logical document structure of a complex text type, namely scientific articles. The algorithm and implementation of a discourse segmenter based on these principles is presented, as well an evaluation of test runs.

    German loanwords are found in many languages in the South Pacific, in particular in those areas which were under German administration before WW I. The Austronesian languages in this area differ greatly with respect to the number of lexemes of German origin. The paper focuses on two languages of Micronesia, namely Palauan, with a comparatively high number of German loans, and Kosraean which had no German influence on its lexicon. The paperconsiders the balance of factors that contribute to the different loanword amounts. That German was taught in local schools for up to two decades did not, by itself, enhance borrowing from German. More weighty factors for the amount of borrowings from German are the length and strength of language contact with English and the use of German as a means of communication in particular settings in the years before WW I.
